Identification of shiga-like toxin Escherichia coli isolated from children with diarrhea disease by polymerase chain reaction ZHU QY, LI LQ, GUO ZB, et al. Lab. Molecular Microbiology, Shanxi Children's Hospital, Taiyuan, O3OO13 (P.R.China) [Abstract]Objective To evaluation of etiology for shiga-like toxin-producing Escherichia coli (SLTEC) in children with diarrhea disease.
Methods We designed and synthesized 3 pairs of primers located in SLT1, SLT2, and eaeA genes of EHEC, while the virulence genes in SLT1, SLT2, and eaeA from E.coli species were amplified by PCR.
Results One strain of EHEC with SLT1, SLT2, and eaeA in 29 reference strains of diarrhea-causing E.coli (DCEC) and 10 strains of other enterobacteria detected by PCR had positive reaction, while all other DCEC and enterobacteria were negative. A total of 474 strains of E. coli isolated from children with diarrhea disease detected by PCR were 20 strains of SLT1 producing E. coli (4.2%) positive, and 7 strains of SLT2 producing E.coli (1.5%) positive, while 74 strains of ESIEC were 15 strains of SLT1 (20.3%) and 5 strains of SLT2 (6.8%) positive.
Conclusion Shiga-like toxin E. coli has been identified as a major etiologic agent of children with diarrhea disease in Taiyuan.

产毒素大肠埃希菌 (Toxigenic Escherichia coli) 是引起人和家养动物腹泻的重要病原体。通常分为三种类型:产肠毒素大肠埃希菌( Enterotoxigenic Escherichia coli, ETEC ) , 产志贺样毒素大肠埃希菌 (Shiga-like toxin Escherichia coli, SLTEC, i.e. Verotoxigenic Escherichia coli,VTEC), 和产坏死性毒素大肠埃希菌( Necrotoxigenic Escherichia coli, NTEC ) [1-3] 。 SLTEC 可产生 3 种毒素类型( SLT1 , SLT2 , SLT2v ) , 主要血清型是 O157:H7, 又叫肠出血性肠炎大肠埃希菌( Enterohemorrhagic Escherichia coli )。 SLT1 和 SLT2 是出血性肠炎和溶血性尿毒综合症的重要致病因子, SLT2v 是猪水肿病的重要致病因子 [4-6] 。 近年来国内又发现了一种新的致泻大肠埃希菌,定名为肠产志贺样毒素且具侵袭力的大肠埃希菌 (Entero-shiga-like-toxin-producing invasive Escherichia coli, ESIEC), 该菌也产生 SLTs 毒素并且具有侵袭力, 是引起腹泻病的重要病原体 [7] 。本研究对太原地区小儿腹泻分离的大肠埃希菌,用 PCR 法作了 SLT1 , SLT2 和 eaeA 毒力基因测定,报告如下。
材料与方法一、• 试验菌株:
1 .标准菌株: EHEC ( O157 : H7 ) 1 株, ETEC 10 株( LT 3 株, ST 4 株, LT/ST 3 株), non-ETEC 2 株, EPEC 16 株,志贺菌 4 株(志贺氏,福氏,鲍氏,宋氏各 1 株),沙门菌 3 株(伤寒,鼠伤寒,肠炎各 1 株),肺炎克雷伯菌 1 株,绿脓假单孢菌 1 株,由中国医学菌种保藏中心和美国 CDC Morris 教授提供。
2 .临床分离菌株: ETEC 32 株, EPEC 8 株, EIEC 2 株,为本实验室从腹泻患儿分离,经血清学与分子生物学鉴定后所保存的菌株。
二.标本来源: 1997 年 1-12 月,在山西省儿童医院门诊和住院的腹泻患儿 1032 例,收集粪便标本于灭菌青霉素小瓶中送检,每份粪便标本分别接种 SS ,麦康凯,庆大,和山梨醇琼脂平板等 4 种培养基,作肠道病原菌常规分离培养。
