【摘要】 目的
评价全自动血培养仪(Vital-200)临床应用情况。
方法
采用回顾性分析，对Vital-200全自动血培养仪检测4 287份标本，检出阳性的时间、细菌种类及阳性率进行了评估。
结果
Vital-200 最短检出时间2小时， 最长166小时，24小时以内阳性者为55.5％，48小时以内阳性者74.8％，72小时以内阳性者86.2％，细菌种类61种，阳性率10.82%；
结论
Vital-200无论从出现阳性的时间、检出细菌种类包括营养条件高的苛养菌阳性率均明显高于本室往年BACTEC-660的结果，而且操作简便、结果快速、准确。
Clinical application and evaluation of an automated blood culture system

【Abstract】 Objective
To evaluate an automated blood culture system.
Methods
A total of 4 287 blood samples were detected by the Vital-200. Time to positivity, the types of microorganisms recovered, and the positive rate were analysed retrospectively.
Results
The shortest time to positivity was 2 h, and the longest 166 h. The cumulative positive rates were 55.5% within 24 h, 74.8% within 48 h, and 86.2% within 72 h. There were 61 types of microoganisms recovered. The positive rate was 10.82%.
Conclusion
The Vital-200 in time to positive, those of the type of microoganisms including fastidous bacteria, and the positive rate were better than those of the BACTEC-660 used in our laboratory.

Blood culture Organism 血培养方法的优劣直接关系到对菌血症的准确诊断、及时治疗。随着计算机技术的发展，血培养方法也由手工、半自动发展至今日的采用计算机进行全自动监测，提高了阳性率，缩短了检测时间。我们对Vital全自动血培养仪检出阳性的时间、细菌检出种类及阳性率进行了临床评价，现将结果报告如下。
材料和方法
一、材料
1.标本来源：1995年6月至1998年6月间来我院就诊的病人（以住院病人为主）。无菌采血5～10 ml注入Vital-200相应配套厌氧或需氧血培养瓶中送检。
2.仪器：Vital-200全自动血液培养监测仪、Vitek-ims全自动微生物自动诊检仪，均为法国生物梅里埃（bioMerieux ）公司产品。
3.培养基：血培养瓶为仪器配套产品，基础为胰酶大豆消化汤，辅以酵母浸液、聚茴香脑磺酸钠及其他生长因子，其他细菌分离鉴定培养基为自制。
二、方法
1.将标本直接上机培养监测：该仪器温度保持在36℃，每15分钟自动监测1次，以培养基内荧光剂分解造成荧光值下降来指示细菌生长，经计算机判读给出阳性信号，阳性者取出转种，分离鉴定。培养满5天未显示阳性者，先将报告单以无菌生长发出，血培养瓶仍继续监测，第十天阴性者，用血平板盲传1次，放36℃24或48小时。在继续监测期间出现阳性及时传种或盲传有细菌生长者，鉴定后再补发报告。
2.细菌鉴定： 对仪器检测显示为阳性的标本及时转种血平板、中国蓝平板、巧克力平板（CO2环境）并放置36℃ 培养18～24小时，厌氧菌培养转种厌氧菌培养基并放澳索德（OXOID）公司生产的厌氧缸内36℃培养48小时，用传统法［1］或Vitek-ims仪器鉴定。
结果
1.阳性率及检出阳性时间： 用Vital-200检测4 287瓶标本，分离出464株菌，阳性率为10. 8％。 最快检出时间2小时（大肠埃希菌），最长检出时间166小时（非发酵菌），24小时内检出的占55.5％，48小时内74.8％，72小时内86.2％；若除真菌外，24小时内占61.6％，48小时内占83.5％，72小时内占91.5％，各菌属检出情况见表1。
